What is an Annual Wellness Visit (AWV)?

Covered by Medicare, this once-a-year wellness visit gives you and your primary care provider the chance to assess your overall health, examine any risk factors, discuss existing conditions + how to prevent new ones, and create or update your Stay Well Care Plan for the upcoming year. 

Think of it as a one-on-one health planning session for the next 12 months, which will also help us determine how often you should be seeing your Village Medical care team.

During your visit, we'll take a big picture look at your health and create a written preventive plan for continued wellness by focusing on:

  • Your medical, surgical, and family history
  • Height, weight, and blood pressure measurements
  • Fall risk, memory problems, and depression
  • Current medications + possible interactions
  • Cancer and chronic condition screenings + health risk assessment
  • Immunizations (i.e flu, pneumonia) 
  • Advanced care planning (if needed)

Annual Wellness Visit vs. Annual Physical Exam:

Here are the key differences between an Annual Wellness Visit, which is covered by Medicare at no cost to you, and a Comprehensive Physical Exam, which is not covered by Medicare.

Can I combine my Annual Wellness Visit with a regular check-up?

Yes. We're happy to address additional concerns during your AWV. Just keep in mind, if you and your care provider decide that it is best to address a medical issue that is outside of the AWV services, a regular visit may be billed with standard out-of-pocket costs based on your insurance.
