We take coordinating your care seriously. At Village Medical, we keep track of the treatment you receive in our practice, via a virtual visit, or for people with certain conditions at home. You will have a unified care team who understands your health history - including any chronic conditions.
Your Annual Wellness Visit.
Your Medicare benefits include a fully covered Annual Wellness Visit every 12 months. This visit helps your primary care team at Village Medical develop a personalized health plan for you. We'll review your medical history discuss current providers and prescriptions, and conduct routine tests plus a cognitive assessment.
Your primary care team is your partner in health - and not just when you're sick. We prioritize preventative care to help you stay healthy by offering services such as immunizations, regular checkups, and exams and tests if needed.
